TMD patient declining orthodontics- other options?

I have a patient with internal derangement, but she only wants palliative care. She clenches, so a flat plane is contraindicated and a deprogrammer would give her a posterior open bite. What would you recommend for a patient who will only commit to a nightime appliance and declines orthodontics? Thank you.
