Dentists in the future must consider restructuring their practices towards helping their patients achieve optimum health. This is accomplished in 3 ways.

  1. 70% of children have a malocclusion. Over 95% of children treated early with functional appliances can be done without the extraction of permanent teeth or surgery.
  2. 35% of the adult population has TMD. TMD must be treated prior to any restorative, prosthetic or orthodontic treatment.
  3. 50% of the adult population snore and 20% have sleep apnea. Oral appliances are much better accepted by the patients compared to CPAP.

Pre-Aligners & Treatment Options

Today, many patients prefer clear aligners rather than fixed braces to achieve straighter teeth. Clear aligners can be utilized in Class I skeletal cases with moderate crowding. By using pre-aligners (functional orthopedic appliances) Class II, Class III and more crowded cases can be converted to routine cases suitable for clear aligners as finishers. Pre-aligners can correct transverse, vertical, antero-posterior problems, overjet, overbite, and molar distalization much more efficiently than clear aligners. Patients and clinicians both benefit from superior results while significantly reducing the number of clear aligners.

Youth Airway: Problems & Resolutions

American Dental Association policy statement in 2017 states that dentists must screen and treat to attain an ìoptimal physiological airway and breathing patternî. Nasal breathing, functional airway, and restorative sleep trump all therapies and precedes malocclusion correction. Dento-facial orthopedics is optimal during early childhood well before permanent teeth have erupted. Appropriate treatments will help correct/manage breathing and sleep disorders, mouth breathing, habitual snoring, teeth grinding, bed wetting, aggressive behavior (ADHD), altered jaw growth, and learning disabilities. Orthopedic growth of the maxillo-mandibular complex allows for the best jaw development with nasal airway and proper tongue posture/function. It is better to shape the childís developing facial structure to optimize functional breathing and sleep capabilities, than to repair them as adults.
